Shipment Preparation

When you have an order ready to be shipped to the Luxottica Lab Services, please follow these steps to ensure quickest service:

  1. Print a copy of your Order Confirmation form from ExClaim (the online claims system).
  2. Wrap this printed Order Confirmation copy around your frame.
  3. All frames should be grouped into one shipping container.
  4. Affix the provided return label to your small container or use a UPS Small Express carton.

    NOTE: Please be sure to record your tracking number in case you need to reference order with Customer Service.

Scheduling a UPS On-Call Air Pick Up

When you have completed your shipment preparation:

  1. Call UPS at 1-800-742-5877 to schedule the pickup.
  2. State you need an ‘On Call Ground Pickup’ or ‘On Call Air Pickup (depending on your label).
  3. Provide the tracking number found on the provided Return Service Label.
  4. Provide the Origin Address (your address).
  5. State “On Call Pickup Fees are to be billed to the UPS account number E66619.”
  6. Provide the destination address and phone number as:

UPS will then establish a time frame for the pickup. Tender the carton to the driver. There should be no cost to you for the shipment or scheduled pickup.

Reordering a set of Return Labels

  1. Call Luxottica Lab Services at 1-855-522-4545
  2. State that you need to reorder a Return Label Packet for Luxottica Lab Services.
  3. Provide the following information:
    a. Your account number
    b. Your EyeMed account name
    c. Your full address (Street number, suite number if applicable, city, state, and zip code)
    d. Total number of packets needed
